Membership Cancellation Application Form

Membership Cancellation Terms and Conditions:

I, the undersigned do hereby request that my direct debit membership be cancelled. I understand that a 30 day notice period is required for all cancellation requests whether they are inside or outside their minimum term. I understand that if my membership is within my minimum term a cancellation fee of $150 plus the 30 day notice period is payable or the balance of the membership fees due, whichever is the lesser. If applicable, the cancellation fees will be deducted in total from my specific account on my next due debit date. I understand that my membership cannot be cancelled while my account is in default and I must pay all monies owing before cancellation can take effect.
